The ten outbound agencies worth shortlisting for healthcare, medtech and digital health: what each one actually sells, which model fits which buyer, and the questions to ask before you sign.

No invented meeting counts, no invented pricing, no scores out of five that mean nothing. Where a competitor is genuinely the better call, it says so.
What actually separates agencies in healthcare
Most "best agencies" lists rank on volume. In healthcare that is the wrong axis. Three things decide whether an outbound programme works here:
- Who the agency can actually reach. A CMIO, a director of clinical operations and a hospital procurement lead are three different buyers with three different calendars, and a list built from firmographics alone reaches none of them reliably.
- How the agency handles regulated conversations. Nothing about cold outreach is HIPAA-covered by default; you are not handling PHI when you call a director of IT. But the moment a conversation touches patient data, systems or workflows, a caller who does not know the boundary is a liability.
- How long the cycle is priced for. Health-system deals run quarters, not weeks. An engagement priced against a 30-day feedback loop will be judged a failure before the first procurement committee meets.
Judge a shortlist on those three, not on how many logos are on the homepage.

The bet is that in a market where the buyer is hard to reach and the cycle is long, a senior human on the phone beats volume sequencing, and that you should be able to buy the meeting rather than the activity.
Best for: medtech, health SaaS and digital health teams that have a closer ready and want the cost of pipeline to be a per-unit number. Watch for: it is outbound only. If you need content, inbound or paid, this is not that shop.
2. Belkins
Email-led appointment setting at scale, with deliverability management as part of the product. The strongest content and SEO operation in the category, and a large SDR bench behind it.

3. Leadium
Managed multi-channel SDR campaigns: outbound as an outsourced function rather than a channel. Long track record, structured reporting.
Best for: companies that want a whole outbound department stood up, not one channel run. Watch for: the same retainer question. See the Leadium review.
4. Leadbird
Cold-email-first, built around list quality and sending infrastructure. Leaner than the enterprise shops, and priced accordingly.
Best for: earlier-stage health SaaS testing whether an offer lands at all. Watch for: email alone into hospital systems has a ceiling; the buyers who matter most are the ones least likely to answer a cold email.
5. Growth Engine X
Outbound systems and infrastructure (the sending stack, the data layer, the automation) sold as build-and-operate rather than as booked meetings.
Best for: teams with an in-house SDR function that needs better plumbing under it. Watch for: if you want meetings on the calendar rather than a working machine, this is the wrong shape of engagement.
6. CIENCE
A large, data-led outbound operation with its own research and data product alongside the SDR service.

7. Martal Group
North American sales-as-a-service for B2B tech, with senior reps carrying conversations further down the funnel than a typical SDR handoff.
Best for: companies that want the agency to work the opportunity, not just open it. Watch for: a model that overlaps your own AEs is a coordination problem before it is a pipeline one.
8. Flowd
LinkedIn-led social selling and outbound, built around founder and executive presence rather than a separate SDR identity.
Best for: clinical and health-tech founders whose own name opens doors. Watch for: it depends on someone senior showing up consistently. If that person's calendar is the constraint, the channel is too.

Appointment setting and outsourced SDR with a long operating history and a US-based calling team.
Best for: buyers who want tenure and a conventional, well-worn process. Watch for: a conventional process is a conventional result; ask what they do differently for a regulated vertical.

Global multi-channel outbound with strong coverage outside North America and Europe, including APAC.

How to actually choose
Shortlist three, then put the same five questions to all of them:
- Who exactly will you reach, and how did you build that list? A named sourcing method beats a database logo.
- What counts as a qualified meeting, and who writes that definition? If the answer is not "you, in writing, before we start", the number you are quoted is not the number you will get.
- What happens to a no-show? This is the single fastest way to tell a pricing model from a pricing gimmick.
- How long before the first meeting, and how long before the first opportunity? Two different numbers in healthcare. An agency that gives one number has not sold into a health system.
- Would you turn this engagement down? An agency that has never said no to a bad-fit market will not say no to yours.
